«In 2006, a pathogenic variant of the common intestinal organism Blastocystis was discovered in patients who were experiencing chronic gastrointestinal symptoms. Most species of Blastocystis inhabit humans with no symptoms. The discovery of a pathogenic variant of Blastocystis is significant, because Blastocystis is related to Entamoeba, a similar organism with pathogenic variants that kill over 100,000 people each year. Recent research has shown that Blastocystis infections may be undetectable using existing clinical methods. Medical case reports from the Middle East, Europe, and United States suggest that infection with this variant may already be widespread and misdiagnosed as one of several functional disorders. HYPOTHESIS: A more virulent or transmissible type of Blastocystis emerged in the Middle East in the 1980's, and was transmitted to Europe and the United States by military and more significantly vacation and business travel. The lack of adequate tests made it impossible to detect the infection. Transmission to the larger population resulted in rising inflammatory bowel disease (IBD) rates in Europe in the 1990's. The relationship between IBD and autism is explored, along with the possibility that the same pathogen causes both conditions. SUPPORTING DATA: Serological and epidemiological findings are presented supporting the hypothesis. Blastocystis survives sewage treatment, shows low host specificity, and can be spread by many animals. Several communities which have been studied due to high autism rates are located close to rivers which receive large quantities of sewage effluent, such as South Thames (England), Olmsted County (Minnesota, USA) and many communities in Oregon (USA). CONCLUSIONS: Scientists from other countries represent the first line of defense against emerging infectious diseases, but their publications on Blastocystis are not well known in the United States and Europe. With the publication of corroborating research by Western scientists in core scientific journals, it is hoped that an appropriate response from the public health system will be forthcoming. Investigation into the existence of infection in the groups mentioned with sensitive and specific tests should be performed. Such tests could include a serum antibody test and a Polymerase Chain Reaction test specific to the pathogenic variant.»

«In recent years, it has been postulated that tramadol, used mainly for the treatment of moderate to severe pain, might display a potential as an antidepressant drug. The present study investigated the effects of acute and repeated tramadol administration on the binding of [3H]RX 821002, a selective alpha2-adrenergic receptor ligand, in the rat brain. Male Wistar rats were used. Tramadol (20 mg/kg, i.p.) administered acutely (single dose), at 24 h after dosing, induced a significant decrease in the alpha2-adrenergic receptors in all brain regions studied. The most pronounced effects were observed in all subregions of the olfactory system, nucleus accumbens and septum, thalamus, hypothalamus, amygdala, and cerebral cortex. Repeated treatment with tramadol (20 mg/kg, i.p., once daily for 21 days) also induced statistically significant downregulation of [3H]RX 821002 binding sites in the rat brain. However, the effect--although statistically significant--was less pronounced than in the group treated acutely with the drug. Since drugs such as mianserin and mirtazapine are potent antagonists of central alpha2-adrenergic receptors and are effective antidepressants, it is tempting to suggest that, in addition to other alterations induced by tramadol, downregulation of these receptors may represent a potential antidepressant efficacy. On the other hand, one should be careful to avoid the treatment of chronic pain with tramadol in patients already receiving antidepressant drugs. Tramadol-induced downregulation of alpha2-adrenergic receptors--when combined with ongoing antidepressant therapy with drugs, which themselves inhibit serotonin reuptake or are antagonists of alpha2-adrenergic receptors--might cause threatening complications.»

«OBJECTIVE: The purpose of this study is to give a systematic review of change of weight associated with commonly used psychotropic drugs. METHODS: Mainly based on a MEDLINE-supported review until April 2005, data from clinical studies with antidepressants, anticonvulsants, mood stabilizers and neuroleptics were scanned for change of weight during treatment. RESULTS: Among antidepressants amitryptiline and nortriptyline have the highest incidence of weight gain followed by imipramine. Maprotiline and mirtazapine have an intermediate weight increasing potential. SSRI (except paroxetine) and MAOI had no or only slight weight inducing effects. In contrary, bupropion was associated with weight reduction. Regarding mood stabilizers and anticonvulsants, a marked gain in weight with lithium and sodium valproate was reported frequently. With gabapentin and vigabatrin a slight to moderate gain in weight was found. Minor changes of weight were found with carbamazepine and lamotrigine. Treatment with topiramate and felbamate reportedly lead to weight loss. The atypical neuroleptics clozapine and olanzapine were frequently related to a strong gain in weight followed by risperidone. Quetiapine has intermediate effects. Stable weight was found with aripiprazole and ziprasidone. A gain in weight is less frequent with older/typical neuroleptics. CONCLUSION: Beside some methodological restrictions like inconsistent information of weight changes (e. g. percent vs. mass) and the small sample of available long term studies, this review specifies the incidence of weight changes for commonly used psychotropic drugs and might be helpful to look for alternatives.»
